Forms are available to download and print, or to fill out online.
Pay registration fee:
Payment can be made by credit card, PayPal, check or cash.

Mail your check to Worcester Chamber Music Society, 323 Main Street, Worcester MA 01608
In-person – bring your payment to registration (cash or check made out to “Worcester Chamber Music Society”)

Instrument Loan Forms
All participants will receive an instrument to use for as long as they are active in the program. The instruments are in good condition, and we expect students to care for them and do their best to prevent accidents. We ask that parents oversee the care of the instruments by providing safe storage space at home.
